Transitioning from clinical setting to case manager or nurse navigator

I have worked in the clinical setting (mostly dialysis) for the last 8.5 yrs. Due to some health related issues, I believe moving to case manager or nurse navigator would be a positive move for me. Any advice on how to break into a new field like this is appreciated. Most places require case management experience. If I can’t get anyone to hire me without experience, how do I get experience?

Don’t move without experience, first learn from you seniors or ask your doubts directly from the experienced people, then evaluate yourself that can you do that case management or nurse navigator job ? after that go for it if you think you can do & and believe in yourself.
