Need help with comfort care documentation


I’m reaching out to other nurses at other hospitals to see how you document when giving patient’s opioid/benzo medication while on comfort care. Our orders state to give PRN opioid when patient exhibits pain, air hunger, dyspnea. Does anyone know of the best pain tool for patients on comfort care (think all patients verbal and non-verbal)? FLACC? 1-10? MBPS? Also, does anyone use a validated tool for air hunger that has numbers to it? Or do you just describe air hunger and dyspnea as accessory muscle use, moaning, grimacing, and frowed brow? Does anyone use a validated anxiety scale to administer benzos for anxiety in patients on palliative comfort care?
